The mission of the Brazos Valley Health Partnership is:
To support the health resource commissions and their communities in improving health and well-being.
Through centralized representation, the Brazos Valley Health Partnership will develop collective strategies that, implemented locally, will leverage and cultivate resources to improve access to services in the Brazos Valley.

2011-2012 Strategic Goals:
1. Increase BVHP's presence regionally as the centralized body representing the rural counties, particularly in the planning for and the development of community health resources.
2. Continue to pursue financial and in-kind resources to support the local health resource center transportation programs.
3. Identify obesity prevention activities which can be customized and implemented in local communities and pursue seed funding to initially support such activities.
4. Continue to develop BVHP in financial management and structure in order to pursue funding for local strategic priorities.
The BVHP's first initiative was to establish an access to care model in rural communities that would be referred to as a "health resource center." BVHP members worked in conjunction with the Center for Community Health Development to finalize the "one-stop shop" health recourse center model and to secure seed funding to create health resource centers in four rural Brazos Valley communities.
Through a Healthy Community Access Program grant, the BVHP worked with local communities and area service providers to develop the health resource centers where multiple providers could share the overhead costs of service delivery and offer services on at least an itinerant basis in each rural community.
The Madison Health Resource Center in Madisonville, the Burleson Health Resource Center in Caldwell, the Leon Health Resource Center in Centerville, the Grimes Health Resource Center in Navasota, and the Burleson Health Resource Center in Somerville, all opened between 2003 and 2006. By 2006, each county's commissioners court had also appointed a county health resource commission to oversee the on-going operations, development and evaluation of their center.

The Burleson Health Resource Center's mission is to:
•Serve residents of Burleson County who are in need of assistance in accessing services by providing information, referral, advocacy, transportation and health resource coordination services.
•Respond to the needs of service providers, educators, health professionals and the court system in accepting referrals of individuals in need of service beyond that which is able to be provided by the referral source.
The Burleson County Health Resource Commission (BCHRC) was initially appointed by Burleson County Commissioners Court May 2005. The BCHRC currently has 9 members. The goals of the group for the upcoming year are to increase access to services and improve the health of Burleson County Residents.

The Leon Health Resource Center's

The Leon County Commissioner's Court created the Leon County Health Resource Commission (LCHRC) in June 2005. The 23 member commission's priority has been to further develop the LHRC by increasing access to services. In 2007, Leon County was awarded a Rural Health Network Development Grant by the Federal Office of Rural Health Policy. With this funding, they are able to partner with Texas A&M University.
